The gross growth pattern is seen here. Note the lobulated growth and apparent satellite lesions. As different islands of malignant cells are growing at different rates, it has a tendency for a cauliflower-type growth pattern.

The histology of I-CAC (cholangiocarcinoma) is a combination of glandular elements and variable amounts of fibrotic tissue and mucin. This explains its usual hypoechoic nature on US, low attenuation on CT, and high signal intensity on T-2 weighted MR.
